The following is a summary of the details we received.<\/p>\n<p>The 2540-10 is effective for cost reporting periods beginning on or after 12\/01\/2010.  For any short period cost reports beginning on or after 12\/01\/2010 and ending 11\/30\/2011, providers will continue to file on the form CMS-2540-96.
